Laparoscopic Surgery - For Ovarian Cyst

 
Surgical treatment of cysts of the ovary involve removal of the cysts alone, preserving the ovary (Ovarian cystectomy) or removal of the ovary together with the cyst (oophorectomy). Technically it is easier to remove Ihe fallopian tube together with the ovary, a procedure called salpingo-oophorectomy.

1. Ovarian cystectomy

a) View of right ovary enlarged by the presence of a cyst b) Cautery of surtace of ovary to reduce bleeding when the initial cut is made
c) Incision of ovary with laser in illustration d) often scissors or cautery is used just as effectively
e) The cyst contents are emptied with a suction device f) The cyst wall is dissected
free and removed
g) The ovary is washed after cyst removal to identify bleeding points h) The bleeding points are
cauterised to secure
hemostasis (stop bleeding)
i) Often the ovary falls together nicely, but if it does not sutures are put in to reconstruct the ovary. j) Ovary at end of procedure

The above is done only when we are reasonably sure that the cyst is not cancerous. If there is a high risk of cancer it may be wise to remove the whole ovary. place it securely in a bag and remove it without spilling the cyst coments. Cystectomy can also be done and the cyst removed intact, avoiding spillage as in the following series of video clips showing laparoscopic ovarian cystectomy for a dermod cyst.
